They could of, but why should they? Should everything be cutsie wootsie?

The fact that it has some realism gives it exposure such as this, which in turn gives the song exposure, which in turn makes them money.

If this was done with merely a fluffy toy style heart it wouldn't get nearly the exposure because that sort of thing has been done to death.

It's great exactly as it is. There wasn't any arterial spray when he cut into his chest, so it's not an all-out gorefest. Just enough blood and organs to grab your attention. Perfect.

And besides, the heart was so big that it had to be a cow heart. If they really wanted to drive the point home they should have used a real human heart. I think any organ donor would be happy to know that his heart was being put to good use in a music video.

The "goryness" of this video is besides the point completely. It makes excellent points on the emotional aftermath of break ups. Really clever, great track
